NAMES OF PERSONS AGE 70 OR OLDER PRESUMED DEAD WITHOUT HEIRS OR BENEFICIARIES OR TO HAVE
ABANDONED ACCOUNTS IN THE WISCONSIN RETIREMENT SYSTEM
Wisconsin State Statute 40.08(8) requires this notice to the persons listed below who have WRS account balances. Benefits are
payable to the person named, or the heir upon application and satisfactory proof of entitlement. Abandoned accounts may be
forfeited as provided by law.
Application forms and information may be obtained by writing the Department of Employee Trust Funds, P.O. Box 7931, Madison,
WI 53707-7931. Inquiries should include participant’s complete name [including published name and former name(s)], date of birth,
Social Security number, and year the name was published.
