The City Of Fox Lake will receive and
accept bids ONLY through QuestCDN.
com via the online electronic bid service
(QuestvBid) for the construction of 2020
Water Main Loop Improvements until January 29, 2020 at 10:00 a.m. All bids will
be downloaded and publicly read aloud
at MSA Professional Services, Inc., 201
Corporate Drive, Beaver Dam, WI 53916
at that time.
The work for which bids are asked includes the following approximate quantities: extension of 1,000 L.F. of 10” water
main by means of open trench and/or horizontal directional drilling methods as well
as related work including erosion and traffic control, site restoration & cleanup, and
related miscellaneous construction.
The BIDDING DOCUMENTS may be examined at the offices of MSA Professional
Services, Inc., Beaver Dam, Wisconsin.

No proposal will be accepted unless accompanied by a certified check or bid
bond equal to at least 5% of the amount
bid, payable to the OWNER as a guarantee that, if the bid is accepted, the bidder

and bond within 15 days after the award
of the contract. BIDDER is required to deliver the original certified check or bid bond
within the 72 hours of bid opening to MSA

will be returned to the bidder as soon as

Pursuant to Section 66.0903, Wisconsin
Statutes, the minimum wages to be paid
on the project shall be in accordance with
the wage rate scale established by Local
wage rates.
OWNER reserves the right to waive any
informalities or to reject any or all bids.

All agencies, groups or individuals disagreeing with this decision are invited to
submit written comments to the above address. Such written comments should be
received on or before February 7, 2020.
All such comments will be considered, and
the UGLG will not request the release of
federal funds or begin the project prior to
such date.
On or about February 8, 2020, the UGLG
will request the Division of Energy, Housing and Community Resources (DEHCR)
to release Community Development Block
Grant (CDBG) funds under Title I of the
Housing and Community Development Act
of 1974 (PL 93-383) as amended for this
project.
The UGLG is certifying to DEHCR that it
and its chief executive officer, in his/her official capacity as Mayor, consent to accept
the jurisdiction of the federal courts if an
action is brought to enforce environmental
review responsibilities, decision-making,
and action; and that these responsibilities
have been satisfied. Upon certification, the
UGLG may use the CDBG funds, and DEHCR will have satisfied its responsibilities
under the National Environmental Policy
Act of 1969. DEHCR will accept an objection to the release of funds and certification
only if (a) the certification was not executed by the chief executive officer or other
officer of the UGLG approved by DEHCR;
or (b) the UGLG’s EA indicates omissions
of a required decision, finding, or step. Objections must be prepared and submitted
in accordance with 24 CFR 58, and may be
addressed to the Division of Energy, Housing and Community Resources, Attention:
Environmental Desk, P.O. Box 7970, Madison, WI 53707-7970.
Objections for reasons other than those
stated above will not be considered by DEHCR. No objections received after February 24, 2020 will be considered by DEHCR.

an Environmental Assessment (EA) for
the following project. The EA is on file at
the Beaver Dam Municipal Building and is

Project Name: S. Spring Street Improvements 2020
Project Location: S. Spring Street (from E.
Mill Street to Cooper Street), City of Beaver
Dam, Dodge County, Wisconsin
Estimated Cost of Project: $3,060,343
Nature/Purpose of Project: The City of
Beaver Dam, Dodge County, Wisconsin
will be committing Community Development Block Grant funds to undertake the
project known as the S. Spring Street Improvements project. The proposed project
includes the replacement of aged sanitary
sewer, water main, storm sewer, service
laterals, manholes, inlets, and appurtenances and roadway reconstruction including curb, gutter and sidewalks.
The UGLG has determined that such request for release of funds will not constitute
an action significantly affecting the quality
of the human environment and, accordingly, the UGLG has decided not to prepare
an Environmental Impact Statement (EIS)
under the National Environmental Policy
Act of 1969 (PL 91-190).
The reason for such decision is because
the project is limited to the reconstruction
of existing facilities. None of the agencies
with who have been consulted noted any
negative impacts and the project is expected to have only beneficial impacts on the
